Suggestions for Completion of Application Forms
Page 1 (Application)
There are three lines beginning just below the heading on the form which require an entry by the applicant.
(1). The first check or X is required to identify the type of membership for which you are applying. These categories are addressed here . Simply place a check mark or an X to the right of the appropriate category (regular, associate, or social) of membership, in the space provided, for which you are eligible.
(2). The second check or X is required to positively inform the MCCA whether or not you desire that your name be published in the MCCA directory. The MCCA will not publish your name in the directory if you do not wish it to be published. If a check mark or X does not appear just to the right of Yes, your name will not be published until you inform the MCCA, in writing, to do so. Please note that the MCCA directory is only sent to members of the Association, and no list or roster of the membership is provided to any other organization except to the company who is engaged to produce a memory book of a reunion. The company hired to produce a reunion memory book is informed in writing that those names and addresses are not to be used for any purpose other than to inform the membership about the reunion book and details for ordering, and is instructed to destroy the list when its purpose has been served.
(3). The third check or X is required to identify the period of membership for which you are applying. You save money if applying for a period of either 3 years or 5 years. When your application is processed, the amount of the check and the position of your check mark or X (to the right of either 1 year, 3 years, or 5 years) should match. If it does not, you will receive correspondence requesting that you identify the period for which you intended to join. This will delay processing of your application.
The next block of required information, your name and address, is self-explanatory. We would appreciate a full nine digit Zip code if it is available to you.
The next block pertains to active military service.
(1). In order to insure that we have accurate information readily available with which to respond to any official queries as to how many and what percentage of our members are "war veterans" under the definition of the Internal Revenue Service, we have identified the six service periods during which individuals on active duty qualify as "war veterans." The six "service periods" are listed below. Please circle all periods which apply.
(2). The date of application is the actual date you complete the form.
(3). The branch of service for all military should be filled with either USA, USAF, USMC or USN followed by an "R" if you performed duty in the reserves. For those who are applying for social membership, please place an S (Spouse) or W (Widow(er)) in the space followed by the appropriate branch of service enclosed in parentheses.
(4). For both regular and associate membership applicants, please provide the MOS under which you are making application.
- For social membership applicants, please provide, in parentheses either the MOS under which your spouse is/would be eligible or your spouse's membership number if your spouse is/has been a member of the MCCA. The MCCA membership number cosists of 2 letters, a dash, and 4 numbers followed by a dash and the year of expiration of the membership. The 2 letters and the first 4 numbers are sufficient to identify the spouse.
(5). The current status is self-explanatory. Just place a check mark or X in the space to the right of the appropriate status.
(6). The highest rank held is self-explanatory. Social applicants please identify your spouse's highest rank in parentheses.
(7). The qualifying activity/duty space should be filled in with the name of the organization (activity) and identity of the specific billet (duty) you are using to claim your eligibility. (e.g., for Regular: 2dRadBn/Ops Chf, or CoA, MSB/Analyst, etc.; for Associate: 1stRadBn/MT Chf; or 2dRadBn/S-4, etc.) For social applicants, if you have identified your spouse's MCCA ID in the MOS space above, no entry is necessary. If you have not, please provide the information, in parentheses, that would qualify your spouse as either a regular or associate member.
(8). To complete the space requiring inclusive dates in qualifying activity/duty (Month/Year), please give the beginning and ending month of the period during which you filled the billet identified in the qualifying activity/duty space on the previous line. Social applicants may leave this space blank.
The signature blank is the certification that all of the material you provide on this page (and those that follow, if you choose to complete them) is complete and accurate to the best of your knowledge. The application can NOT be processed unless your signature appears in this space. The remainder of the page is for MCCA record keeping data.
Completion of Page 2 of the Forms
As you will note, the page begins with background information on the particular page and those that follow. We urge you to complete as much of the information as you can for the reasons stated.
Please note that if you do not place an X or check mark in sthe space after Yes following the question about publishing your spouse's name in the directory, your spouse's name will not appear in the directoy until you have authorized us to do so by separate correspondence.
There is a small bit of space near the bottom of the page. Please feel free to add any comments you may wish to make.
Military Service History
The completion of pages 3 and 4 of the forms assists us in correlating individuals with locations and times; thereby perhaps helping us all to refresh our memories about events, crucial and otherwise, and with whom we shared them. An applicant for Social membership need not complete these pages; however, she/he is welcome to do so. In doing so, please indicate if the information applies to the spouse or the applicant.
In filling out the Military Service History, please be as explicit as possible about your specific unit, the base or station at which it was located, your rank/ranks during the period and the periods (beginning and ending month and year of the duty). If you need more space, please do not hesitate to reproduce page 4 and change the number to read 5 and so on.
On page 3, we have provided space for any medals/awards you have received. If you need more space, do not hesitate to use the space below the lines or reproduce page 3 and renumber it as 3a.
